The Farnam Center served as a health office for the Dells and Baraboo community until the opening of the House of Wellness clinic. The House of Wellness clinic first obtained accreditation with the Accreditation Association for Ambulatory Health Care (AAAHC) in 2003. Recertification of this facility was awarded in 2006. In 2010, the House of Wellness clinic received its recertification of accreditation for another three years.
The House of Wellness clinic is open Monday - Friday, 8:00 a.m. to 4:30 p.m., except tribal holidays. (See Pharmacy for adjusted hours.)
Arrive 15 minutes (30 minutes for new patients) prior to your scheduled appointment. This allows time to fill out the necessary paperwork. Remember to bring in your insurance card, Tribal ID card or other picture ID card, and social security card.

We have doctors, nurse practitioners, registered nurses, laboratory technologist, pharmacist, pharmacy technician, optometrist, dentist, exercise physiologist, and registered dietician. We also have many support staff such as those working in medical records, billing, registration and scheduling for full clinic services.
Doctors: Our doctors can provide a wide range of services and health care. These include family practice and pediatrics.
Nurse Practitioner: Our nurse practitioner can provide a broad range of health care services, which include:
- Take the patient's history, performing a physical exam, and ordering appropriate laboratory tests and procedures.
- Diagnosing, treating, and managing acute and chronic diseases.
- Providing prescriptions and coordinating referrals
Nurse Educator: Nurse educator provides a variety of patient education materials on topics such as: diabetes, pre-natal education, asthma, smoking cessation, cholestral, and high blood pressure. The goal is to provide information and support to patients with health concerns, enabling them to manage a condition and live a life of optimal health.
Registered Nurses: Our registered nurses assist the medical team in providing care and health education to all clinic patients. These nurses are available to answer your question or to refer to a matter to health care provider.
Optometry: The Optometry Clinic is staffed by an optometrist and an optometric technician. Optometry clinic offeres comprehensive eye examinations and contact lens services. Adjustments and minor eyewear repairs are also available. A wide variety of frames and lens are available through the dispensary, at a reasonable cost. There are elibility criteria for using our optometry services. Call our dental/optical registration staff for assistance.
Pharmacy: We have a full-time pharmacist and certified technician operating our clinic pharmacy.
Podiatry: The podiatrist on staff is able to perform most services from routine care to surgery. Podiatrist has limited hours. For services, costs of services and hours of availability, call the clinic patient registration staff for assistance.
Laboratory: Laboratory services are provided at the HOW Clinic. Some routine lab tests are done at the clinic. Those tests that cannot be performed here are sent to an outside reference laboratory.
X-Ray : Radiology services are open at the HOW Clinic on a limited basis. Those services that cannot be performed here are referred to St. Clare Hospital in Baraboo.
Exercise Services: The exercise specialist or physiologist can work with you on a routine basis to help you learn the important steps to a successful exercise program. Often referrals to our exercise physiologist are provided by the clinic physician. Meet with your health care provider and talk about a referral for exercise plan and program.
Dental: The Dental Clinic can provide exams, x-rays, teeth cleaning, sealants for children, fillings, dentures, and crowns. There is an eligibility criteria for using our dental clinic. For service eligibility, services available, costs of services, call the dental/opitical registration staff for assistance.
Nutrition Services: Our registered dietician can provide medical nutrition therapy for health issues such as: diabetes, alcohol recovery, pregnancy, high cholesterol, high blood pressure, digestive problems, and weight management. Services include, but not limited to, meal planning, shopping & healthy cooking tips, label reading and dietary management of diabetes. Clients may be physician or self-referred.
