          Application for Employment (Complete & Submit On-line, pages 1-4, Page 4 must be printed and submitted manually with your signatures as required for your application to be accepted and processed by the Department of Personnel.)  If you are using version 9 or later of Acrobat Reader you will need to disable PDF/A View Mode in your client in order to view PDF forms correctly. 

To configure Adobe Acrobat Reader 9 to properly view PDF Forms:

• Browse to the form above, Application for Employment.
• Right-click on the document and choose "Page Display Preferences" from the menu
  
• Choose the Documents category at the top-left, and then change the value of the PDF/A View Mode drop-down box to 'Never'.
